What is Answer Engine Optimization?
Answer Engine Optimization — AEO — is the practice of setting up your online presence so that AI tools recommend your business when people ask for help.
When someone types "Who's the best dentist near me?" into ChatGPT, the AI doesn't show a list of 10 results like Google does. It picks 3 to 5 businesses and recommends them by name. If your practice isn't in that short list, your potential customer went somewhere else without ever knowing you exist.
AEO is how you become one of those 3 to 5 names.
How is AEO different from SEO?
SEO (Search Engine Optimization) helps you rank on Google's list of results. AEO gets you into the direct answer AI gives people. They're related but they work differently.
With SEO, you're competing for position on a page of options. With AEO, you're competing to be the recommendation itself. AI doesn't show ads, doesn't show 10 blue links — it picks a few businesses and tells people to go there.
The biggest surprise from our research: the things that matter for AI recommendations are completely different from what matters for Google rankings. Domain authority — the metric most SEO agencies obsess over — has almost no effect on whether AI recommends you.
What actually matters for AI recommendations?
We've studied this across thousands of AI responses in multiple cities and industries. Here's what the data shows:
- —Website structure is the single biggest factor. How your pages are organized, titled, and connected matters more than anything else.
- —Dedicated pages for each service or condition you handle are essential. Generic 'services' pages don't work.
- —Clear, well-organized information that AI can easily read and extract from.
- —Consistent business information across the web.
- —Content that directly answers the questions people actually ask.
Why does AEO matter right now?
AI search is growing fast. More people are asking ChatGPT, Perplexity, and Google's AI for recommendations every month. But almost no local businesses have set up their websites for it.
In our studies, less than 2% of practices had the structural elements AI needs to recommend them. That means the first businesses to get this right in each market will have a head start that's very hard to catch up to.
This isn't theoretical. We've analyzed over 19,000 AI responses across 41 cities. The patterns are clear and the opportunity is real.
How does AEO work in practice?
Getting your business recommended by AI comes down to three things:
1. Website structure. Your site needs to be organized in a way AI can understand. That means clear page titles, proper heading hierarchy, and logical navigation. AI reads your site like a textbook — if it's messy, it skips you.
2. Content depth. You need dedicated pages for each major service you offer. A chiropractor needs separate pages for sciatica, back pain, sports injuries, and so on. A dentist needs pages for implants, cosmetic work, emergency care, and more.
3. Technical signals. Structured data, consistent business information, and making sure AI crawlers can actually access your site. These are invisible to visitors but critical for AI.
Is AEO right for my business?
AEO works best for local service businesses where people ask AI for recommendations — healthcare practices, professional services, home services, and similar industries.
If your customers might ask AI "Who's the best [your service] near me?" then AEO matters for your business.
The good news is that the changes needed for AEO often improve your Google rankings too. You're not choosing one or the other — you're adding a new channel that also strengthens your existing ones.
